Registered Nurses at Support Worker 4U
A Registered Nurse ( RN ) at Support Worker 4U can help support people with disability with specific health and clinical care needs. RN’s are responsible for assessing, planning, and the evaluation of nursing care for patients. They perform tasks such as coordinating the care of patients, providing treatments and therapies, including medication, and monitoring patient responses to both the treatment and the care plan. RN’s also are responsible for answering questions and providing information to patients. RNs can provide treatments, and therapies such as administering medications and monitoring responses to treatments and care plans.
0114: Community Nursing care – Registered Nurses ( RN’s )
At Support Worker 4U we can provide community nursing with supports to participants within their home or living setting these are participants who require higher levels of care and assistance. Our Registered Nurses can assist with bowel care, wound care, catheter care, tracheostomy management, ventilation support, subcutaneous injections, PEG and NG Tube feeding and the administration of medication. Usually involving advanced care or training supplied by the RN.
Catheter Care and Management: Ensuring proper care, insertion and maintenance of urinary catheters.
Wound Care: Managing and treating wounds to promote healing, we have a clinical nurse educator who educates on these topics and can carry out wound care.
Tube-Feeding Assistance otherwise known as Enteral feeding: Supporting individuals who require tube feeding.
Respiratory Support: Assisting with breathing difficulties and respiratory conditions, this includes but not limited to home, Tracheostomy, CPAP or ventilatory support.
Complex Bowel support- This may apply to participants’ who are prone to constipation or have faecal incontinence, this could be related to nerve damage, cerebral palsy with gross motor function classification 3, 4, 5; some types of acquired brain injury; some neurological conditions such as stroke, autism, and where the support plan involves non-routine treatments eg, prn medications.
Subcutaneous injections- this could be for various reasons the most common being diabetes.
Stoma Care: Providing specialized care for stoma patients and education if required
Monitoring Vital Signs: Regularly checking blood pressure, pulse, temperature, and blood sugar levels.
Education for Clients and Families: Providing information and training on health management.
Visiting Nurse Services: Conducting general nursing assessments in clients' homes.
Development of Care Plans: Creating personalized care plans tailored to each individual’s needs.
Medication Setup: Organizing and preparing medications for administration.
Continence Advice and Management: Offering guidance and support for continence issues.
Palliative Support: Delivering compassionate care for those with life-limiting illnesses.
Overnight Nursing Care: Providing nursing support during the night.
Care After a Hospital Stay: Assisting with recovery and rehabilitation at home.
Diabetes Management: Monitoring and managing diabetes care plans.
Medication Administration/Supervision: Administering medications and supervising their use.
Dementia Management: Supporting individuals living with dementia.
Personal Care Assistance: Helping with daily personal hygiene tasks, such as showering that may involve some more complex care due to a stoma or catheter.
Support for Attending Appointments: Assisting clients in getting to and from medical appointments.
Post-Operative Care to Rehabilitation: Supporting recovery from surgery and facilitating rehabilitation.
24-Hour Nursing Care: Offering round-the-clock nursing support for those who need it.
Community nursing care is essential for promoting health, independence, and quality of life for individuals requiring varied levels of support.
0107: Daily personal activities
This service assists participants to ensure their personal care and hygiene is always upheld. Participants may be required to complete personal care tasks such as showering, toileting, changing etc, this is done in the participants home. These support items provide a participant with assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. This service is like the ‘Assistance with Daily Personal Activities’ service however it is much more complex. Workers will be required to assist with bowel care, wound care, catheter care, tracheostomy management, ventilation support, subcutaneous injections, PEG and NG Tube feeding and the administration of medication.

This support item, which includes mentoring, peer-support and individual skill development, is designed to establish volunteer assistance within the participant’s home or community to develop skills.
